Housing First Service

													
Who are we?
The Housing First team provides secure and settled housing as the first response for individuals with multiple and complex individual needs.  Eligibility is simply that the individual is willing and committed to their own tenancy agreement, and the home is provided for as long as needed.
Housing First is offered to individuals who have been homeless before but who struggle without additional support. 
What do we do?
The Housing First team provides housing stability, and long-term individualised support, allowing an individual to build a positive and sustainable future. Individuals accessing the service will design their own support, at their own pace, and how they want the service to help. 
 Aims and Objectives
Our aim is to provide tailored support within the context of a secure tenancy. This will include:
•	Improved knowledge, skills and support to maintain and manage a tenancy
•	Remove the risk of homelessness
•	The reduction of personal risk and the promotion of personal safety
•	Increased awareness of health and wellbeing and self-management
•	Support independence through self-confidence and self determination 
•	Reducing anti-social behaviour or involvement in the Justice System
•	Improve access to community and social supports and reduce isolation
•	Support recovery and harm reduction through access to addictions or mental ill health services
•	The provision of a clear, easily understand package of care
How do we do it?
We work with individuals to identify goals that are important to them; and engage with a wide range of health, social care, housing, and support services to ensure the right assistance is in place. There is no fixed end date; we acknowledge that recovery takes time and varies in relation to individual need and experiences. 
How can you access us? 
This is a professional only referral service. Referrals are received through the Housing Needs Assessment Officers within Falkirk Council Housing Service and if eligible Transform Forth Valley provide the required supports.
